How to register

The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.

You can register online by clicking here. Once you have submitted the forms, you will be emailed instructions on what you need to do next.

It takes approximately 7 to 10 working days for a registration to be processed.

The Registration department opens Monday to Thursday between the hours of 10:00am – 12:30pm. The department is closed on Friday. Any enquiries or new registration forms can only be taken during these times. Forms can be collected at any time during our opening hours, or can be completed online at any time.

Pre-Registration

Patients who have never registered with a GP in the UK before will need to bring a copy of their passport and include on the registration form the date that they came to live in the UK.

  • Check you are within our catchment area – click here.
  • We do not accept patients outside our catchment area.
  • You can register by completing the registration forms available to print below or from reception. You will need to complete this by hand and ensure you sign the form and bring it to the surgery with 1 form of photographic ID (e.g. passport, driving license etc.) and 1 form of proof of address dated within the last 3 months (e.g. utility bill, bank statement etc.) to support your application.
  • You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

You can also apply for an online account at reception, which will allow you to book GP appointments and order repeat medication. There is no form to complete for this, but photographic ID and proof of address are required to confirm identity before username and password can be issued. This can only be done once you have been fully registered as a patient at the practice.

If you have no fixed abode

Unfortunately we are not able to accept registrations for patients of no fixed abode. The surgeries in Swindon who can are listed below:

Registration Forms

Our registration forms are available to print below. They are also available from Reception. These must be completed, signed and returned in person to reception during the registration department open hours, along with your photographic ID and proof of address. For children, please ensure you bring their red book. Each family member must have their own registration form.
